To apply for a University Card, applicants must check boxes on the online Card Request form to confirm their agreement to the terms and conditions for using University and Bodleian library resources.
Card request sent directly to the Card Office
If you are part of a card administration team and you are completing a card request form on behalf of an applicant (ie using the 'Directly to Card Office' option in the Card Request Management app), please ensure the applicant confirms that they agree to the University Card and Bodleian declaration text provided on this page.
University Card declaration
I wish to apply for a University card. I understand that the personal information I have provided may be used to determine access to a range of University and College resources (e.g. premises, libraries, and computing services). I understand that the information I provide (including the photograph) will be collected and used in accordance with the University’s staff privacy notice and the General Data Protection Regulation (GDPR). I undertake to obey the rules of any institution or facility to which I gain access by use of the card I receive. I agree and acknowledge this.
Bodleian declaration
I hereby undertake not to remove from the Library, or to mark, deface, or injure in any way, any volume, document, or other object belonging to it or in its custody; not to bring into the Library or kindle therein any fire or flame, and not to smoke in the Library; and I promise to obey all rules of the Library.
